invoiceRequestManagers V4.0, V4.1, V4.3: new feature "pseudo service code" and bug fixes

New builds of the listed invoice request managers have been generated due to the new feature "pseudo service code".
By using pseudo service codes the underlying validators can be controled and its default behaviour can be changed.

This is especially needed for swissDRG services of maternity clinics or for the relocation case.

The new builds of the modules are available as installers and/or netUpdate
